Submit Initial Report for Review and Discussion   
If you observe a violation - fiscal or otherwise - determine the area of abuse and fill out the associated confidential report form on the OffRecord website and submit it to OffRecord via the online system or via Fax if you prefer. The online system is an SSL encrypted link. We will review your report and call you within 48 hours to share our initial reactions to your case.

 

What kind of evidence is necessary? Be sure to document whatever evidence you have on hand to prove your point, and if you would like to submit it with your initial documentation this will simply strengthen our ability to assess the viabilty of reaching a successful monetary settlement. Finding the highest quality lawyer and ensuring he is successful in bringing the guilty to justice will require you have the highest quality evidence possible. Memorandums, signed documents, emails or other forms of evidence which confirm the violators knowledge or desire to cover their trails are particularly useful. Hunches or suspicions not backed up by clear evidence will make the cases far less viable and likely difficult for OffRecord to consider pursuing.

Tips

Safeguard your Passport.  You must present your passport to the Customs and Border Protection officer upon arrival in the United States.  Carry your passport—do not pack it in your checked luggage. We recommend you make a copy of your passport and put it in a separate place as a safeguard in case it is lost or stolen.

Traveling with Children.  All children, including infants, must have their own passport or Trusted Traveler Program for U.S. entry. Carry documents for traveling with minor children.

  • If you are escorting a minor child without the parents, have a letter from both parents indicating that you have permission to travel with the minor.
  • If the child is accompanied by only one parent, the parent should have a note from the child's other parent. For example, "I acknowledge that my wife/ husband is traveling out of the country with my son/ daughter. He/She/ has my permission to do so."
  • If a single parent has sole custody, a copy of the court custody document can replace a letter from the other parent.
