Should I Fill-Out the Short Form or Full Form Report?

OffRecord would generally prefer you do the Full Form.  This provides us the most detailed amount of information to determine the validity of your case.  Perhaps the most important part is for you to explain what you have in terms of evidence to help legitimize your claims.  Simply having a "hunch" or "suspicion" that a division in your company or team member has committed foul play or done something illegal is not enough. It is important to be able to provide verifiable evidence that the illegal activity occured. Also, only having some verbal confirmation based upon a statement from someone else will not in all likelihood, be enough.  The burden of proof will be upon us to clearly demonstrate in writing, photo or video than an individual was violating a law, or committing gross negligence, either knowingly or unknowingly. While this is not required in all cases, it increases the likelihood of getting an appropriate outcome on the case, and will likely effect OffRecord's and the prospective legal counsel's interest to committing on contingency to get involved. 

The Short Form is merely an abreviated form that does not have all the data the Full Form requests. In most circumstances, the Short Form is a satisfactory starting point for determing what next steps, if any, are required. Although the Full Form is preferred, we respect your time and can begin our work on the Short Form if this is easier for you.  Completing the Full Form however will from time to time cause individuals to disclose data and added info which they did not originally realize would be very valuable in the process.  Sometimes, after receiving the Short Form, OffRecord may still request at a later date that you complete the Full Form.  

The Quick Question form is simply to enable you to get a quick reply to a short question perhaps about the forms or the process. It is not meant as a mechanism however to be considered as your submission for review by our team to review the case and consider a call or face to face meeting.
